Facing a serious illness can make everyday challenges even more overwhelming.


Transitions™, a free community support program sponsored by Hospice of the Calumet Area, is dedicated to improving the quality of life through compassionate care for individuals and families facing a life-limiting illness. Decisions are being made and life is changing.

Transitions joins clients and caregivers at a time often filled with confusion and uncertainty.

Our support team brings relief by providing companionship, assistance with tasks and referrals to community resources. Funded with the help of community donations, Transitions services are provided at no cost.

Our Transitions Program helps clients:

  • Navigate the healthcare system.

  • Help clients and families develop a care plan.

  • Offer advance planning conversations.

  • Assist with goals of care conversation.

  • Support clients and families to lessen their burden.

  • Identify community resources available.

  • Facilitate access to appropriate services.

  • Support and respect client and family privacy and confidentiality.
