Development/Fundraising Assistant
Hospice of the Calumet Area seeks a dynamic individual to join our development team. Responsible for administrative support of development department, donor relations, event planning, database management and gift acknowledgement management. Work closely with the Director of Development and our Friends of Hospice auxiliary.
The ideal candidate is self-motivated, with strong community relations and writing skills; proficient in Microsoft Office applications, database management and social media platforms. Fundraising and/or event planning experience.
Position is Full-time, Monday – Friday, travel between Munster and Porter offices. Requires flexibility to work occasional evenings and weekends based on special event schedules.

Social Worker
Hospice of the Calumet Area is looking for a dynamic Full-Time Medical Social Worker who is passionate about quality end-of-life care and our Cherishing Life philosophy.
Responsibilities include:

Conduct comprehensive psychosocial assessments to determine patient and family needs and develop appropriate treatment plans.

Engage in case management activities, ensuring clients receive the necessary resources and support services.

Collaborate with hospice interdisciplinary team to coordinate patient/family care.

Provide education and support for patient/family on coping strategies related to terminality, anticipatory grief and death.
-
Maintain accurate medical documentation in compliance with HIPAA regulations, including patient records, notes, care plans and advance directives.
Qualifications
The successful candidate will possess:

Master’s Degree from an accredited school of Social Work.

Two years of experience in health care; hospice or home care preferred.

IN/IL Clinical Social Work License.

Hospice Admission Coordinator
Work closely with families, discharge planners and referral sources to coordinate referrals through the admission process. Work collaboratively with our admissions team to ensure that all eligible patients receive timely care. Position is full-time M-F with administrative on-call rotation.
Must be a registered nurse with a minimum of 3 years nursing experience including 1 year of acute care hospital experience. Hospice experience required; leadership experience with strong interpersonal communication skills preferred.
Position is Full-time; current IN and IL license required.
