Home Health Aides/CNAs
Department: Clinical Team
Hospice of the Calumet Area is seeking dedicated, compassionate PRN Certified Nursing Assistants (CNAs) to join our team. These positions offer the opportunity to make a meaningful impact through home care visits and shifts at our inpatient William J. Riley Memorial Residence.
Qualifications

Minimum 1 year of CNA experience required.

Home care or hospice experience preferred.

Must be listed on the Indiana CNA Registry.

Hospice Medical Social Worker – Full-Time
​Department: Clinical Team
Are you looking for an opportunity to use your outstanding clinical skills and make a difference? Hospice of the Calumet Area is seeking a compassionate, dynamic Medical Social Worker who is passionate about quality end-of-life care and our Cherishing Life philosophy.
Key Responsibilities

Conduct comprehensive psychosocial assessments to determine patient and family needs and develop care plans.

Provide case management services, ensuring clients receive appropriate resources and support.

Collaborate closely with the interdisciplinary hospice team to coordinate patient/family care.

Offer education and emotional support for patients and families on coping strategies related to terminal illness, anticipatory grief, and loss.

Maintain accurate medical documentation in compliance with HIPAA, including patient notes, care plans, and advance directives.
Qualifications
-
Master’s Degree in Social Work from an accredited program.

Minimum 2 years of healthcare experience preferred.

Current IN/IL Clinical Social Work License required.
