Transitions™ joins clients and caregivers at a time often characterized by stress and confusion. Consideration of options for care, decreased energy or lack of knowledge of community resources can be daunting.

The Transitions™ support team can minimize the difficulties of this challenging time by providing companionship, assistance with tasks and linkage to community resources.

We're here to help.

Transitions™ services are provided at no cost. Community donations help fund Transitions™.

The Transitions™ staff can provide:

- Help with locating community resources to assist you at home

- Help in making decisions about options for care

- Help with coping with the challenges of a life-limiting illness

A Transitions™ volunteer can provide:

- Companionship visits

- Respite breaks for caregivers

- Help with errands and meal preparation
