The Hospice care team works closely with the patient’s family and physician to develop a scheduled plan of care.
General Process 1. The Hospice admission team is available to meet with the family to explain hospice services and answer any questions. 2. Once the patient and family chooses hospice care, and initial assessment of the patient’s needs is performed. The hospice team collaborates with the patient and family to initiate the plan of care with symptom management and pain control especially emphasized. 3. Regular visits are scheduled with a professional nurse. The frequency of those visits can be adjusted depending on the patient’s need. At each scheduled visit: · A complete patient assessment is done. · Medications are reviewed and adjusted as needed. · Caregivers are assisted in learning treatments and patient care. In addition, our nursing staff is available 24 hours a day, 7 days a week. 4. Home health aides, pastoral care, social services and volunteers may be scheduled to visit based on patient or family need. |
